It can be obtained by completing a quest given by the Grave Keeper in HellGrave Village.

Talk to the HellGrave Keeper in HellGrave Village.

Select the Quest for Pulsating Heart. (Check the monster list here).

Complete the quest objective, usually involves defeating 100 monsters inside HellGrave Undergrounds
Area: HellGrave Village

Return to the NPC to complete the quest.

Receive 1× Pulsating Heart as your reward.
